Reporting a claim during normal business hours:
Claims should be reported to your insurance agent. He/she will complete the proper loss report and submit it by fax (920-829-5154) or e-mail (claims@maplevalleymutual.com) to our claims department.

Our normal business hours are 8:00 to 4:30, Monday through Friday.
Reporting a claim after normal business hours and on weekends:
If you have an emergency claim that needs immediate attention such as a serious water damage loss, fire loss or other significant loss, please contact one of the following people at the number(s) listed:
