• ACH Payment (Automatic withdrawl from your checking or savings account.) This option will automatically withdraw the amount of your billing on the due date:

1) Under the DOWNLOADS Section of this website, print a copy of our Electronic Funds Transfer form
2) Complete the form and attach a voided blank check
3) Mail to:

• Make a One Time Online Payment and/or Review Policy Information:

Contact Maple Valley Mutual (Phone 800-236-2753) and request that your password be entered into our system.  You will need to select a password with up to 8 characters before you call us.  This password can also be used for future payments.  After we have entered your password in our system, please click on the link below to log in and make your payment or review your policy information.  You will need your policy number and password.
